You can now place your Plastic Card re-orders through dbs4pos.com! Simply fill out the form below and hit SUBMIT at the bottom to send in your order...

New orders or re-designs have a one-time design & layout fee of $150.00. Gift and Membership (VIP) cards are $1.00 per card if under 500 cards are ordered. Order 500 or more and the price per card drops to .75 per card!

Manager cards (or Employee Access Cards) are $5.00 each and come with a belt-reel to make sure you and your staff never leave your card behind!
